Description:Streetwork Project is the oldest street outreach program for homeless and street-involved youth in New York City. The program has grown from its beginnings in 1984 as a storefront project to encompass both outreach and comprehensive harm reduction services. The core service is long-term, one-on-one counseling, which is offered in a non-judgemental and holistic manner utilizing a harm reduction approach that focues on building trust, fostering self-esteem, exploring options and choices, and empowering youth to change their high-risk behaviors.

In order to help a survivor of violence, we must support their right to make their own decisions and choose their own path based on what is most important to them. This is the foundation of client-centered practice. At Safe Horizon, Client Centered Practice means supporting clients as they make difficult choices in their lives around their safety.

Responsibilities:• Conduct psychosocial interviews and prepare reports to support housing and SSI disability application
• Coordinate and manage all SSI disability applications;
• Analyze and use client medical and educational records to assist in establishing SSI eligibility;
• Track clients’ progress through the housing and benefits component;
• Provide vigorous advocacy on behalf of clients within benefits and housing systems; ‘Work in consultation with our psychiatrist to establish clients’ eligibility for benefits and supportive housing;
• Work closely with Housing Coordinator to promote access to supportive housing, HASA housing, transitional housing and DHS shelter and family shelter placement;
• Monitor policy changes that impact client’s eligibility for obtain and navigate supported-income, shelter and long-term housing
• Provide training on resources, benefits and advocacy to case management staff and collaborate with them to support applications;
• Establish relationships with key government agencies and housing and social service providers;
• Participate in advocacy efforts within the broader runaway and homeless youth continuum
• Provide supportive counseling to clients including those presenting multiple and complex problems and severe mental illness;
• Escort clients to appointments with providers and government agencies;
• Provide intake as needed;
• Provide in-house individual crisis intervention and emergency assessments;
• Participate in weekly supervision;
• Provide documentation of all services.


Secondary Job Functions:

• Support the oversight of daily program activities;
• Participate at staff trainings, staff meetings, and staff group process workshops;
• Supervises MSW or BSW level intern

Qualifications:• Understanding of issues confronting population
• Harm reduction approach
• Skill in individual and system advocacy
• Compassionate non-judgmental style

Qualifications:

• Masters in social work or related field
• Three (3) years experience providing services to at-risk youth or other oppressed populations required/or at least 1 year with Streetwork providing direct service care.
• Bilingual (Spanish/English) preferred.
