Description:SHCC is a New York State-licensed outpatient mental health clinic that specializes in the evidence-based, trauma focused treatment of child and adult survivors of crime and interpersonal violence. SHCC is also a member site of the National Child Traumatic Stress Network (NCTSN), a federal initiative charged with promoting the spread and implementation of evidence-based treatments and elevating the standard of care for traumatized children. Our goal is the ongoing integration of the best available empirical research with measures of clinical expertise and an understanding of our client’s needs and values. As an evidence-based center of excellence we must enhance our ability to track client outcomes and to use this data to improve the quality of our services. The Records Clerk plays a key role in maintaining the clinic’s compliance with all mandates (federal, state, city, funder, etc.), particularly as we expand into satellites.

Responsibilities:Essential Job Functions:

• Works closely with the Intake Coordinator and the Licensed Quality Improvement Specialist to maintain the clinic’s status with NYSOMH, DOHMH, and SAMHSA.
• Files all clinical documents on a daily basis
• Assesses new client intake documents to determine if the file is accurate, complete, and ready for conversion into an official client record
• Assesses all closed client records to determine if the file is accurate, complete, and ready for archiving
• Intercepts all records requests and complies in accordance with client consent and all other applicable regulations
• Maintains the organization of the chart room
• Coordinates annual file purging and archiving
• Assists with the maintenance of the SHCC client database

• Ability to step back and question the reasoning behind procedures
• Excellent organizational skills
• Ability to work both independently and collaboratively in a rapidly changing and diverse environment
• Demonstrated ability in proactive problem-solving
• Data base management experience (or Basic Excel skills)


• High School Diploma or GED preferred


• Professional, internship or volunteer experience working with survivors of crime and interpersonal violence preferred
• Spanish preferred
• Thorough knowledge of NYSOMH and DOHMH regulations preferred
